Restaurant Rewards Programs and Birthday Clubs Around Ventura County

My family is always out and about, dining out at various Ventura County area restaurants. I've got young kids so in general I'm not looking for "high end" meals...just decent food at reasonable prices. It's nice finding Kids Eat Free deals but there is also a huge array of loyalty/rewards programs. Thus, I felt compelled to compile the most comprehensive list of Rewards Programs and Kids Birthday Cubs in and around Ventura County ever known to human civilization.
This list includes rewards programs where you actually get something for signing up...a free burger, a coupon to save money on future purchases, etc. I didn't include email programs that provide mainly promotional updates.
